Care Team

Individual Care Plans
An individually tailored Care Plan is developed for you and with you or your care representative following formal medical / nursing / personal / social assessments. The Care Plan covers all aspects of your care, from assistance that may be needed for everyday activities - for example
mobility, personal hygiene (e.g. help with showering) - to specific nursing needs (e.g. dressings to a wound). An important part of the Care Plan is your social and personal needs. 

The Care Plan is developed around evidence-based best practice for care of older persons and as much as possible is focused on maximising your independence, quality of life and personal preferences. The Plan is reviewed daily and a formal reassessment is conducted at least every 4 months with you and the multidisciplinary team.

 

Team Nursing
The Director of Nursing leads the Care Team in The Marlay. The Care Team consists of the Assistant Director of Nursing, Clinical Nurse Managers, Nurses, and Health Care Assistants. Each person within the Care Team is responsible for a specific group of residents. All staff are trained in Care of Older Persons and ongoing training for our staff is at the core of our service.

 

Doctors/General Practitioner (GPs)
Your current GP may be willing to continue to provide a GP service to you in the Nursing Home - however some GPs may decide not to continue this service if for example, the Nursing Home is too far from their practice. If this is the case, we can facilitate a local GP service for you or you can select another GP of your choice who is willing to visit you at the Nursing Home. This will be arranged in consultation with you or your care representative. In addition, we also have the services of a Medical Officer who visits the home twice a week. Out of hours we use the Doctor on Call service.
