Prior to admission each resident has an assessment in their place of residency, at home or in hospital, to establish their care and social needs. From this assessment the Care Planning process commences and upon admission, the resident’s plan of care is further developed and drawn up with input from the resident, their next of kin, the nursing staff and GP.
